In the context of the 4th Consortium Meeting, Research associates from AUTH Medical Physics and Digital Innovation Lab had the chance to present the VITALISE Project and its available key features for research and Well-being. During an interactive co-creation session, participants from both projects, FOR21 & VITALISE, discussed the ways that new technology solutions can be used to support transitional care for palliative patients & patients with multimorbidities.
